I am applying for a Nurse Practitioner position part time or full time. I am very flexible with salary expectations.

I am a Nurse Practitioner with 11 years of direct patient care experience.

I currently hold an active AZ NP license AP 5545 Exp 4/1/18 and DEA number - MW 1468722 in good standing.

October 2017- present: Corizon Health; Tucson Prison Complex

Sept 2011- Oct 2017: Matrix Medical Network. Clinical Service Manager for West Region. Along with managing up to 40 other nurse practitioners

Do in home assessments of medical histories

Aug 10-Sep 2011: Fortis College Nursing Instructor

I helped organize and implement a new nursing program for Fortis College in the Salt Lake City area. I taught multiple subjects and clinical rotations. I held faculty positions including Staff development, quality improvement and program development for state certification of the Nursing program.

Oct 08-Oct 2010: Urology clinic of Utah Valley, LLC.

I saw up to 28 patients a day for all urology related issues. I conducted pre-op and post-op interviews. I assessed and admitted patients to the hospital as necessary.

Oct 07-Oct 08: IMFT. “Occupational Health”

Independent Nurse Practitioner. FNP I had an acute patient load of 12 to 32 patients daily. I treated minor injury and acute illness as well as manage chronic illness patients. I was a central figure in building a new clinic on site and implemented an electronic medical records program for the facility that generated over $870,000 dollars of medical cost savings within the first six months of operation.

Sept 06-Nov 07 Self-employed in a private practice, “House Calls”

I did all my business in the client’s homes. I treated patients from 6 year of age to end of life.

June 06-Sep 07: Ameritech College Nursing Program Instructor for RN’s. I taught clinical labs, skills lab, nursing assessment, pharmacology and substituted for all nursing subjects. In addition to my teaching role I was the only facility nurse practitioner and provide services as the care provider for the students, staff and their families.

Fall 2006: JEAN BROWN RESEARCH

I worked for a few months for Jean Brown as a PRN, NP. The position was upgraded to full time and I could not assume the position at that time.

Dec 05-Sep 06: University of Utah. Inpatient Psych/Med

I was a staff nurse taking care of inpatient Psych patients with medical complications.

2004-2005 Central Utah Surgical Centers

I worked in endoscopy and pre/post-op.

2000-2004 DaVita Dialyses

I delivered outpatient-nursing service to chronic dialysis patients. I supervised a staff of 6 to 8 on any given day, with a load of 50 patients a day. I provided complete patient care; placed IV’s, changed dressings, did complete physical assessments, and administered medication along with all other charge responsibilities. I assumed the senior leadership role for the patient care floor.

1998-2000 Charge nurse, Benchmark Hospital (Psych)

I provided direct supervision of 98 inpatients and supervised a staff of 10-12. I provided direct patient care including assessing, care plan development, medication administration etc.

1997-1998 Gunnison Hospital, Gunnison Utah

I held the Director of Home Health position for one year. I supervised a staff of nine to twelve nurses and a varying patient load. I provided direct patient care as part of my duties. My administrative duties included; QI, budget management, Medicare compliance, ICD-9 education and all direct hire personnel responsibilities.

1988-1996 USN, Navy Nurse Corps

I had many nursing assignments which included Med/Surg, OB/GYN, Labor/Delivery, Psych, Pediatrics and Alcohol/rehab.

My last tour of duty was as a Medical recruiter. During the last year of active duty, I was honored with the Navy Medical Recruiter of the Year.

I was the officer in charge of the North West region, supervising officer and enlisted recruiters in Utah, Idaho, Wyoming, Oregon and Washington State.
