e. Make dose adjustments based on patient specific data
such as renal and hepatic function, drug concentration
levels and other relevant laboratory values
f. Monitor for drug interaction and other disease state
contraindications
2. Provide drug therapy consultation and recommendations
regarding dosage adjustment, drug interactions, drug allergies,
compatibility, and drug bioavailability.
3. Participate in patient rounds with the medical team and provide
therapy consultation and drug information for the team.
a. Review existing orders and recommend appropriate
changes when warranted
b. Ensure all drug allergies have been appropriately
documented
c. Reconcile patients home medication orders with
inpatient orders to ensure there is continuation of
therapy when appropriate
d. Make recommendations regarding interchange of drug
therapy in order to minimize non-formulary orders
4. Serve as pharmacist liaison for the selected medical unit
a. Facilitate communication between nursing and
pharmacy
b. Ensure that nursing staff is using required procedures
for storage, withdrawing, administering and
documenting medication orders.
c. Provide drug information and education for nursing
staff
