Office is based in Texas. Job Description: Gather health related
information such as medical history, conduct physical examination, obtain
specimens, educate patients on their health issues, and provide some advice
on what issues to discuss with their primary doctor during their next
appointment. The house-call visit is part of the care management the
patient receives as a plan member of the insurance company that I am
serving for.
SPECTRUM FOR LIVING, ADULT TRAINING CENTER IN RINGWOOD NEW JERSEY
This is a not-for-profit organization that helps adults with developmental
disabilities attain their maximum potential by providing quality housing
and clinical services in state of the art facilities. The Ringwood Adult
Training Center provides traditional and medical special needs programs.
The center provides traditional day program options along with computer
training and community integration. As a Physical Therapist in the Adult
Training Center, I evaluate clients and provide necessary physical therapy
treatment when it is indicated.

This is a 100-bed capacity skilled nursing facility that provides 24 hour
nursing care, physical, occupational and speech therapy to patients with an
array of cardiac, neurologic, pulmonary, orthopedic, and general surgical
conditions.
Position: Per-diem Physical Therapist: August 2006 to present
Worked on an as needed basis to provide physical therapy evaluation and
treatment to patients.

Besides providing physical therapy evaluation and treatment to patients, as
