Comments

When I was working for John Menser I helped him or did it

myself connect utilities, install satellite, repaired plumbing,

repaired floors, and did all vehicular Repairs/maintenance. I

also drove Mrs. Menser around, saw to it that her medication was

filled and taken properly. As well as saw to all medical

appointments, and financial responsibilities.

When I worked for Country Side Healthcare I was hired as a

Certified Nurses Aid Student. I absolutely had to leave town on

my primary job and miss the second day of orientation. Which was

not allowed. So I was unable to complete the course.

When I worked for Ronnie Veatch. I delivered, sold, built,

and finished oak furniture. I worked for Mr. Veatch on two

separate occasions. But he has passed away since I worked for

him last.

When I worked at Future Force Temporary Services. I worked

as replacement help for company's that had contracted Future

Force to replace help out sick or on vacation.

I studied under a certified mixologist for at least six

months, before I went to work at the Olympic restaurant Mixing

drinks, answering the telephone, taking take out orders, and

cashing out customers. As well as serving alcoholic and none

alcoholic beverages. I also was instructed on how to detect

improper identification, and over intoxication.

When I worked for Camp Grace Bentley. I worked for one

summer. As assistant cook/camp counselor/athletic director. As

assistant cook, I helped prepare all meals. As a camp counselor

I monitored the campers, saw to hygiene and entertained. As

athletic director I scheduled all athletic activities

When I worked for Hank Polaski. He paid me to help out with

his heating and cooling business as well as the maintenance of

his rental property. I helped install ac/heat units, duct work,

cleaned houses, plus numerous repairs, and maintenance.

What is WOTC?

W.O.T.C. stands for the Work Opportunity Tax Credit Program. It is a

Program created by federal law that is administered in Kentucky by the

Office of Employment & Training (OET), a state government agency.

How can the WOTC help me get a job?

The WOTC program helps certain people get jobs by giving employers an

Incentive to hire them. This incentive is a credit against federal taxes

owed

By the employer. The maximum tax credit allowed by the WOTC-for hiring

QUALIFIED EX-FELONS is $2400 per eligible employee hired. Letting

Employers know that they can save $2400 by hiring you may help you to

get a job.

How do I Qualify for the WOTC Program?

There are 9 groups of people that qualify for the WOTC program. One of

these

Groups is QUALIFIED EX-FELONS. A QUALIFIED EX-FELON IS a person who has

Ex-felons date of conviction or release from incarceration.

How does the employer get the tax credit?

In Kentucky an employer must mail to the office of Employment & Training

An IRS Form 8850. Prescreening Notice and Certification Request for the

Work Opportunity Credit, within 28 days of the employee's first day of

Work. The employer must also submit an ETA 9061, individual

Characteristics Form, and proof that the employee is an ex-felon who has

Been convicted or released from incarceration within a year of the hire

date.

The ex-felon may need to provide the employer with such proof. OET

Reviews the forms and documentation. If the conditions are met, OET will

Submit to the employer an EMPLOYER CERTIFICATION FORM. The IRS requires

That the employer have this form before the tax credit may be clamed.
