Qualifications and Capabilities
NFT Specialist/Housing Coordinator-February 2010 through Present, The Information Center, Taylor,
Michigan. Provide services, referrals and assistance to nursing home residents and their families. Provide
resources to participants and their families in order to assist with transition from nursing home setting to assisted
living facilities, semi independent and independent living environments. Conduct initial and reassessments,
plans of care and other documentation requirements in accordance with agency standards. Discuss and promote
program to nursing home social workers, staff, participants and families. Provide housing resources for care
managers, locate and evaluate housing opportunities for appropriateness and suitability based on participant’s
choices and needs.
Program Supervisor-Housing Resource Center- November 2008 through December 2009, Southwest
Counseling Solutions, Detroit, Michigan. Provided supervision to full time and part time staff; providing
clinical and case management services to homeless individuals and families. Provided individual and group
supervision in order to maintain compliance with agency standards, Wayne County Department of Mental
Health and other funding sources. Participated in Blended Management meetings with Southwest Housing
Solutions in order to assist consumers with mental health and housing issues. Assisted with developing program
standards, goals and criteria as well as responsible for submitting reports to HUD and other funding sources.
Responsible for conducting employee evaluations, handling disciplinary issues, conducting chart reviews and
participating in staff development. Also responsible for the development and implementation of a consumer
advisory council and supervising staff who utilize the Illness Management Recovery model.
Clinician/Therapist- Children’s Special Initiative/Youth Assistance Program - June 2004 through November
2008, Southwest Counseling Solutions, Detroit, Michigan. Provide court ordered clinical services to at risk and
adjudicated youth and their families. Conduct psychosocial assessments, develop person centered treatment
plans, participate in case conference, participate in child and family team meetings, attend court hearings and
facilitate psycho educational groups (Anger Management, Life Skills and Trauma) for youth on site and in
schools.
Program Supervisor-Children’s Special Initiative - September 2001 through June 2004, Southwest Counseling
and Development, Detroit, Michigan. Provided supervision to six fulltime professional staff people. Conducted
weekly individual and group clinical supervision, reviewed all treatment plans, Juvenile Justice Plans of Care,
Integrated Plans of Care and all other relevant documentation. Conducted staff evaluations and made personnel
recommendation to Program Director. Participated in quality assurance activities to assure compliance with
Wayne County requirements.

Clinical Therapist-Assertive Community Treatment Team – March 1999 through September 2001, Southwest
Counseling and Development, Detroit, Michigan. Provided services to clientele with severe and persistent
mental illnesses as well as substance abuse problems (dual diagnosed). Provided individual and group therapy,
substance abuse counseling, developed treatment plans, completed psychosocial assessments, assisted clients
with personal, medical and psychiatric needs and completed documentation in accordance to agency and CARF
standards.
Probation Officer – January 1998 through March 1999, Third Judicial Circuit Court, Detroit, Michigan.
Responsible for providing services to adjudicated youth ages 12-19 who had been diverted to the Intensive
Probation Unit for the purpose of providing clinical services. Also responsible for conducting psychosocial
assessments, developing treatment plans and utilizing the Child Adolescent Functional Assessment Scale
(CAFAS).
School Social Worker/Counselor - March 1997 through January 1998, Project Education-Willow Run School
District, Ypsilanti, Michigan. Responsible for providing counseling services to youth (ages 15-19) who, for
behavioral and academic reasons, are not able to attend their neighborhood schools. These services included
individual, group and family counseling, peer mediation, conflict resolution, self-esteem building, providing
guest speakers and referrals, career and college counseling and assistance with classroom assignments.
Clinical Social Worker - September 1996 through February 1997, Lula Belle Stewart, Incorporated, Detroit,
Michigan. Responsible for providing clinical services to children referred for specialized foster care assistance
and their families, individual, family and group counseling, referrals for psychiatric evaluations and the
development of treatment plans for children and families.
MYC-Community Program Director/Urban Services Director - - December 1995 through July 1996,
Downtown & Northwestern YMCA, Detroit, Michigan/ November 1991 through December 1995, Downtown
YMCA, Detroit, Michigan. As Community Program Director, responsible for organizing and supervising youth
summer camps, after-school programs, character development programs and supervision of staff/volunteers. As
Urban Services Director, responsible for the operation of the 120-bed Transitional and Residential Housing
Programs, implementing policies and procedures, supervising support staff and social work interns, maintaining
department budget and maintaining appropriate documentation and statistical reports for the YMCA and various
funding sources.
