Team Leader, Community Based Flexible Support Program
Managed multidisciplinary team of 11 staff who provided services for
seriously mentally ill clients in the Cambridge Somerville community. Team
was formed under a new model of supportive care funded by Department of
Mental Health. Ran day to day operations, coordinated with other
providers, provided direct services to clients.. Program closed as of July
31, 2010.
BOSTON HEALTHCARE FOR THE HOMELESS, Boston, MA 2008-2009
Manager, Case Management Department
Supervise Case Management staff at Barbara McInnis House, a residential
medical respite program for homeless and marginally housed individuals.
Case Managers develop discharge plans in conjunction with medical team and
work with patients to complete other non-medical issues. Assessed length
of stay issues, screened patients for placement in long term care, worked
with finance department on insurance issues, worked with IT to design new
electronic medical page for case management services.
CODMAN SQUARE HEALTH CENTER. Dorchester, MA 2007-2008
Coordinator Suboxone Treatment Program
Coordinated newly state funded office based opioid treatment program
using.Suboxone. Provided assessment, support and treatment for patients
with opioid addiction. Worked closely with medical and behavioral health
services to insure provision of comprehensive services.
LYNN COMMUNITY HEALTH CENTER, Lynn, MA 2005-2007
Manager HIV Clinical Program
Provided management of Ryan White Title III HIV care program, working
closely with both clinical and clinical support team (nurses, case
managers, social workers) to coordinate care for over 100 clients.
Implemented and managed adherence program with a home visit component;
Expanded nutrition and mental health services. Developed and managed
Continuous Quality Improvement Program for HIV+ clients producing quarterly
reports on selected quality parameters. Manage stated funded Service
Collaboration Grant for the North Shore with the goal of improving
collaboration among providers on behalf of HIV+ clients.

emergency medical leave. Worked closely with primary providers, community
case managers and Infectious Disease specialists to provide continuity of
care and medical case management services for a multi-lingual caseload
(Portuguese, Spanish, Haitian, English) of patients.

Managed Ryan White federal grant to provide services in support of primary
medical care including comprehensive case management, on site mental health
services, and nutrition support, including budget management, year end
reporting, grant renewal. Collaborated on federal SPNS grant for adherence
support.. Supervised peer program, mental health program, and program
support services. Continued to provide direct patient case management
services.
RN Case Manager, HIV - 1997 to 1999
Responsible for coordinating with primary care providers and clinical
support team to ensure quality, comprehensive medical care to growing HIV+
caseload of approximately 175 patients receiving care at inner city health
clinic. Managed expanded access program for new medications. Provided
telephone and urgent care triage, adherence support, case management
services and patient teaching, Focus on maintaining client linkages with
multiple providers and supportive services,

MEDFORD VNA, Medford, MA 1994-to1995
Home Care Nurse and social worker
In split position, maintained two caseloads one to provide skilled nursing
services and one to provide social work services to patients. Skilled
nursing services included: initial assessment and development of
appropriate plan of care; implementation of care plan, coordination of
services with other disciplines family and physician; and maintenance of
appropriate documentation. Social work services included psycho-social
assessment, evaluated community need, developed and implemented
appropriate care plan, including supportive counseling, case management,
referral to appropriate community services and re-assessment of the
situation.
ELDERLINK, ADULT FAMILY CARE PROGRAM, Somerville, MA 1991-1993
HIV Coordinator and social worker
Developed and managed Medicaid funded Adult Family Care Program, which
provided financial and social support to People Living with AIDs and their
caregivers. Service provided was in home support for clients with end stage
HIV by a nurse and a social worker. Established criteria for admission,
developed forms, wrote marketing material developed and maintained
information files, and provided direct services. Services provided
included: intake, assessment, crisis management, case-management,
information and referral, client support, case collaboration, and
consultation. Outreach conducted with providers in Greater Boston.
Provided on-going counseling support around issues of relationship end-
stage disease, loss and grief as well as providing support and linkages to
appropriate community services.
I have also provided case management services in a community mental health
center,worked as a protective service worker with children involved in
abuse and/or neglect situations and with clients who are mentally retarded.
I have been an office manager in 2 small software companies; and have
provided tour escort services on motor coach tours.
