Outpatient Coding: ICD-9-CM, CPT, E/M coding from EMR, fee slips, physician reports.

Analysis, Re-Analysis and Concurrent Analysis of patient charts. Scanning patient records

into EHR program.

Auditing patient records. Query physicians for added patient information to code patient’s

record correctly.

Make newborn patient chart and assign appropriate number sequence.

Maintain patient files per HIPAA regulations. Terminal digit filing reports, color-coded filing,

loose sheet filing in appropriate patient charts.

Answer multi-line telephone, routing calls to appropriate departments or individuals within

medical records. Volume varies from 25+ calls per shift. Answer multi-line telephone requests

from hospital staff, patients, attorneys and physicians.

Prepare individual physician deficiency reports and pull deficient charts for each physician to

complete - one to 200+ charts. Contact 50+ physicians regarding deficient reports on a

weekly basis.

Remove Physician from suspension list as he/she has completed their deficiencies. Contact

the Surgery department to inform them the physician has regained his/her surgery privileges.

ROI: Faxing, mailing patient record requests, per HIPAA guidelines in all medical record

security, requests.
