As a HIM clerk working for POI at Sarasota Memorial Hospital, I
work with doctor's offices, medical facilities, insurance
companies (for discharge date information), and medical
equipment supply companies. In this position, I received
requests for patient information via telephone and fax,
determining HIPAA status of requests, requiring patient
authorization for the release of sensitive information as per
guidelines, and providing fast and courteous turnaround for
medical records. I prioritized requests based on status such as
STAT record requests, communicating with the requester the
timeframe and document information requested, such as H&P,
Consults, labs, etc. Attention to detail and being able to
quickly locate and retrieve the required medical information was
crucial to this position. Customer/Doctor/Facility satisfaction
was of utmost importance, while providing them with the
information needed. The pace was brisk and the volume processed
daily was high. A positive attitude was an asset and a necessity
for working in this environment.

As a medical records processor, I viewed records on a PC, first
verifying that a valid, HIPAA compliant authorization had been
submitted. Next would be viewing the request for date validity
and requested by, fee approval or fee limits, legal
representation when appropriate. I reviewed records making sure
that the correct patient's records were provided and that all
other details complied with the authorization. I viewed and made
sure that legal requests including NOI/SUBPOENAs were adhered
to. If the records were in order and able to be sent, I printed
them for mailing, or prepared them for uploading or downloading
as requested by client. If problems were found with the records,
I returned them to the requester, the field representative at
site, or customer relations with information detailing what
needed to be corrected. I also used the mailing machine,
scanners for records that needed to be scanned into Prism (the
program used at IOD) and assisted co-workers in completion of
daily workload.
Experience: Compositor, Sarasota Herald-Tribune, Sarasota, FL,
January 1998 to November 23, 2008.
While employed at the paper, I worked as an ad compositor for
approximately 6 years using Camex, Multi-Ad Creator II, Quark 4,
Ad Manager, DPS AdTracker, Photoshop, Illustrator, and Adobe
InDesign CS3 with Morcor/XpanceX 5.3.1. During this time I found
and corrected artwork, composed ads that were built to standards
for printing, and worked within the system to release completed
ads within daily deadlines. I worked in a partner's environment
in the Manatee Bureau of the Herald-Tribune for approximately 3
years, having sales staff and prepress in close working
relationship and proximity. I was an integral part of the
process mapping for the bureau partners team. While in the
group, meetings were held with focus on customer satisfaction,
increasing revenue, and quality management. After being moved
back to the main office, Sarasota, in Feb. '08, all ad materials
and correspondence with the remote bureau in Manatee were
accomplished via email using Lotus Notes for layouts, artwork
and insertion orders. I also retrieved ads, PDFs and other
materials electronically from FTP sites and via email from
advertisers.
Prior to ad building, I paginated the classified section of the
paper and sent completed news pages to the plate room via
computer. Part of my job duties involved picking up and
processing artwork and layouts for production in prepress and
maintaining close communication with sales staff. I also
performed inputting of ad copy and proofreading of ads on a
daily basis.
From January 2008 to July 2008 I served on the Culture Council
as a part of a Strategic Council setting used to brainstorm at
the Sarasota Herald-Tribune. During this time period, the
Culture Council formulated, presented and analyzed employee
surveys, conducted employee roundtables, and presented findings
to the Executive Steering Committee.
Ad Layout, Bradenton Herald, Bradenton, FL 1989-1998
At the Bradenton Herald I worked in the mailroom running
inserters, driving forklift and using hand jacks, ran the
bindery stitching TV books, was a team leader overseeing weekend
shifts, used a labeling machine and a quarter fold machine. I
was also responsible for simple machinery maintenance such as
belts, cleaning, adjustments on suction and placement of
insertion wheels and suction bars, strappers and bottom wrap for
newspaper bundles. I received and documented inventory and also
prepared skids for
shipment.
After leaving the mailroom, I moved into the Ad Services area of
Prepress where I retrieved physical artwork for paste up, pulled
and filed tear sheets for billing later to be matched up and
mailed monthly with invoices. I manually laid out the newspaper,
placing ads, receiving input from newsroom and advertising
pertaining to space requirements and placement needs.
Various Hospital Employment to 1989
Working in the Dietary department of hospitals starting in high
school and beyond, I learned a great deal about the medical
environment including direct patient contact, medical
terminology/records, and hospital guidelines. I have been a
lead/relief supervisor overseeing approximately 10-12 employees.
This background aided me in dealing with my husband's severe
health conditions and disability. Before he passed away, I
learned how to administer shots, give wound care, flush shunts,
handle insurance claims and follow through. These opportunities
also gave me the patience needed to maintain a productive and
positive outlook through very trying circumstances. Names of the
hospitals that I have worked at include St.Mary's Hopsital,
Madison, WI; Blake Hospital, Bradenton, FL; Manatee Memorial,
Bradenton, FL; and FHP Hospital, Fountain Valley, CA (this
hospital I actually helped facilitate opening of).
