safety and appropriateness as well as advising on medication selection dosages and
providing drug information. Provide support to “ER” on code green (Pedi-cardiac
arrest). Supervised seven technicians. Worked in an outpatient to provide
pharmaceutical services by checking/verifying prescriptions, interaction with
physicians and counseling outpatients.
Bienvivir Senior Health Services, El Paso, Texas
Pharmacy consultant, October 2003 January 2005 (Part time) Bienvivir Senior
Health Services was established as a private non-profit organization that operates
under permanent Medicare and Medicaid waivers to provide comprehensive health
and health related services to an exclusively frail population that is certifiable for
institutionalization. The model uses a prospective capitation financing system. This
means that the program receives a dual allocation from Medicare and Medicaid for
each participant at the beginning of each month. The program pools all resources and
then provides any services needed by each of the participant on an “as needed” basis.
There is an incentive to keep participants functioning at the highest level possible in
order to reduce hospitalization and institutionalization costs.
Duties of pharmacy consultant included but not limited to:
1 Participate in monthly pharmacy committee meetings
2 New participant medication management: seeing new participant in the clinic,
getting medication and allergy history, communicating/educating caregiver
about formulary, assessing appropriateness of medications, discussing with
medical team and making formulary changes as necessary on initial
medication orders.
3 Participate in care planning, whenever necessary.
4 Consult and help physician in drug selection
5 Analyze monthly drug utilization and cost data, monitor adverse drug reaction
and medication errors, and participate in P & T committee.
6 Work closely with dispensing pharmacy to assure adequate medication
system, participate in projects that assess quality care.
7 Instill evidence based prescribing, helping primary care physician manage
specific diseases/conditions (i.e. anticoagulation, lipid management; asthma,
heart failure etc.)
