Work Environment This role primarily involves onsite work in a home care setting, allowing you to provide therapy in the comfort of our patients' homes.

You will work Monday through Friday, collaborating closely with other healthcare providers to ensure comprehensive care.

Key Responsibilities - Conduct thorough evaluations of patients, including reviewing medical histories, assessing functional mobility, and developing tailored treatment plans.

- Communicate evaluation findings and treatment options clearly to patients and their families, ensuring that they are involved in setting achievable rehabilitation goals.

- Implement evidence-based treatment protocols based on each patient's unique needs and medical presentations, adjusting interventions as necessary based on patient progress.

- Collaborate with interdisciplinary healthcare teams to facilitate comprehensive patient care, recognizing when to modify care plans or seek further medical consultations.

- Complete discharge planning and follow-up care arrangements, ensuring continuity of care and that all patient documentation reflects a clear course of treatment.

- Maintain effective time management by adhering to a structured schedule, attending necessary meetings punctually, and ensuring productivity metrics are met.

- Engage in community outreach initiatives to promote Spring Leaf Physiotherapy's services and enhance awareness of rehabilitation options.

- Contribute to departmental goals and objectives while complying with relevant regulations and standards in the physical therapy field.

- Stay committed to your professional growth by pursuing continuing education opportunities, and mentor aspiring therapists as a clinical instructor.

Patient Population / Client Focus Our patient population primarily includes individuals recovering from orthopedic surgeries, sports injuries, and chronic musculoskeletal conditions.

Qualifications - Bachelor's degree in Physical Therapy is required.

- State licensure as a Physical Therapist, valid in Virginia.

- American Heart Association's Basic Life Support (BLS) for Healthcare Providers certification is mandatory.
