Nurse Practitioner – Skilled Nursing Facilities

Location: Danvers and Sagus, Massachusetts

United Family Medical Center is seeking a compassionate Nurse Practitioner to provide care to residents in skilled nursing facilities in the Amesbury and Haverhill area. This role offers the opportunity to build meaningful relationships with patients and collaborate with facility teams to deliver high-quality care.

Responsibilities

Conduct patient assessments, develop treatment plans, and manage acute and chronic conditions.

Provide ongoing care, follow-up visits, and medication management.

Collaborate with physicians, nurses, rehabilitation staff, and other care team members.

Support transitions of care and communicate with patients and families.

Maintain accurate and timely clinical documentation.

Requirements:

Active Nurse Practitioner (NP) license in the state of Massachusetts

Current board certification (FNP, ANP, or relevant specialty)

Valid DEA license and prescriptive authority in Massachusetts

Experience in primary care, family medicine, or a related clinical setting preferred

Strong clinical assessment, critical thinking, and diagnostic skills

Excellent communication skills and a compassionate, patient-first approach
