Work Environment This position is primarily onsite, facilitating home care services to patients.

Key Responsibilities - Conduct thorough evaluations of patients referred by physicians, identifying their unique needs related to orthopedic conditions.

- Develop, implement, and modify personalized treatment plans based on the patients' progress and physician's guidelines.

- Utilize various evidence-based therapeutic techniques and procedures to achieve measurable improvements in patients' conditions.

- Demonstrate excellent customer service skills while interacting with patients and their caregivers, ensuring they feel supported and informed throughout their healing journey.

- Work collaboratively with multidisciplinary teams, sharing insights and contributing to joint treatment strategies.

- Educate patients and caregivers about ongoing treatment plans, exercises, and rehabilitation strategies to ensure consistency and understanding.

- Meet established utilization goals and other performance-based objectives set by the organization.

Patient Population / Client Focus In this role, you will work predominantly with individuals facing orthopedic challenges, but there will also be opportunities to treat pediatric, neurologic, and sports medicine populations.

Qualifications - A Bachelor's Degree in Physical Therapy from an APTA or CAPTE-accredited institution.

- A Doctorate in Physical Therapy (DPT) is preferred.

- Current State License in Physical Therapy.

- Basic Life Support (BLS) Certification from the American Heart Association.

- Strong written and verbal communication skills to effectively interact with patients, caregivers, and team members.

- Competence in word processing and basic computer skills.

- Demonstrated ability to build rapport and establish trusting relationships with patients, enhancing their commitment to achieving functional goals.

- A genuine desire to make positive connections with both patients and the community at large.

Preferred Experience While we welcome newly licensed therapists, candidates with prior experience in orthopedic rehabilitative care will be given preference.

Familiarity with a wide range of therapeutic modalities and treatment techniques will also be advantageous in this role.
