Responsibilities:

Conduct comprehensive client assessments to identify needs, strengths, and barriers to stability

Develop and implement individualized service plans in collaboration with clients

Connect clients with community resources, social services, and support programs

Monitor client progress and adjust care plans as needed to ensure goal achievement

Maintain accurate and up-to-date case documentation and client records

Advocate on behalf of clients with external agencies, providers, and stakeholders

Facilitate referrals to mental health, housing, employment, and other support servicesRequirements:

Bachelor's degree in Social Work, Human Services, Psychology, or a related field

Prior experience in case management, social services, or community outreach preferred

Strong knowledge of local New Haven and Connecticut community resources

Excellent communication, advocacy, and interpersonal skills

Ability to manage a caseload effectively while maintaining attention to detail

Empathetic, culturally competent, and trauma-informed approach to client care

Proficiency in case management software and Microsoft Office SuiteAbout Us:
