Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team at Titusville Nursing and Rehab. About the Role:

Provide direct patient care to residents in our skilled nursing facility, including administering medications, documenting patient records, and developing individualized care plans.

Conduct routine assessments and evaluations to identify patient needs and develop effective care strategies.

Collaborate with registered nurses, doctors, and other healthcare professionals to ensure seamless resident care.

Maintain accurate and up-to-date records, reports, and charts.

Demonstrate a commitment to patient-centered care, upholding the highest standards of quality, confidentiality, and respect.

Responsibilities:

Provide skilled nursing care to residents, including wound care, medication administration, and respiratory therapy.

Develop and implement individualized care plans in collaboration with the multidisciplinary team.

Participate in quality improvement initiatives and contribute to a culture of excellence.

Maintain a clean and safe environment, adhering to facility policies and procedures.

Communicate effectively with patients, families, and healthcare team members.

Participate in educational and training programs to enhance knowledge and skills.
