As a Licensed Practical Nurse Weekend Baylor (LPN), you will play a vital role in delivering high-quality care to our residents, supporting their physical, emotional, and social well-being.

About the Role:

As a Licensed Practical Nurse Weekend Baylor (LPN), you will work every weekend, providing direct patient care to a mix of long-term and short-stay residents. You will be responsible for administering medications, wound care, and vital signs, as well as assisting with daily living activities such as bathing, dressing, and grooming. You will also be responsible for maintaining accurate and detailed records of patient care and communicating with the healthcare team to ensure seamless care transitions.

Responsibilities:

Provide direct patient care to residents in a skilled nursing facility setting

Administer medications, wound care, and vital signs as prescribed by physicians

Assist with daily living activities such as bathing, dressing, and grooming

Maintain accurate and detailed records of patient care

Communicate with the healthcare team to ensure seamless care transitions

Participate in care planning and development of resident individualized care plans

Collaborate with other members of the healthcare team to achieve positive outcomes for residents

Requirements:

Current LPN licensure in the state of Pennsylvania

1+ year of experience as an LPN

Ability to work every weekend, with occasional flexibility to work weekdays as needed

Strong communication and interpersonal skills

Ability to work effectively in a fast-paced environment

Compassionate and empathetic approach to patient care
