PRIDE Health is seeking a local contract Home Health Physical Therapist for a local contract job in Winter Haven, Florida.

Schedule: Monday–Friday 8:00 AM–4:30 PM

Location: Winter Haven, FL

Coverage Area: Polk County

Contract Length: 10+ Week Contract (Possibility to Extend)

Pay Range: $60.00 - $65.00/hour (W-2) Plus Mileage reimbursement

Mileage Reimbursement Rate: $0.49 mile

*Pay offered is based on experience, expertise, credentialing, and education

Duties:

Perform comprehensive physical therapy evaluations and ongoing reassessments to identify impairments, functional limitations, and safety risks.

Develop, implement, and adjust individualized care plans to improve mobility, strength, balance, endurance, pain management, and functional independence.

Deliver skilled interventions including therapeutic exercise, gait training, transfer training, balance activities, stair training, and home safety education.

Educate patients and caregivers on home exercise programs, fall prevention, assistive device use, and energy conservation techniques.

Complete accurate and timely documentation (evaluations, visit notes, progress reports, and discharge summaries) using EMR systems.

Coordinate care with physicians and interdisciplinary team members, including nurses, occupational therapists, speech therapists, and home health aides.

Communicate changes in patient condition, safety concerns, or barriers to progress promptly to the care team.

Participate in case conferences, care coordination activities, and team collaboration.

Ensure compliance with CMS regulations, infection control standards, and agency policies.

Manage a mobile caseload efficiently while meeting productivity and documentation expectations.

Adapt quickly to new environments, workflows, and patient populations as a travel clinician.

Requirements:

Education: Master’s Degree

License(s): Active Physical Therapist license

Certification(s): Current CPR certification

Years of experience: At least 1 year of Geriatric home health experience and experience with HomeCare HomeBase (HCHB) is a plus

Note: Travel contracts are also available if you reside more than 50 miles from the specified location - Travel Pay Package: Up to $2,899/week gross (includes weekly non-taxable stipend)* Plus Mileage reimbursement.
