Hospice RN Case Manager - Field / Visiting Nurse

***NOTE: THE PAY IS $55-100 PER VISIT, depending on the type of visit. Plus mileage. IT'S NOT PER HOUR**

Local Hospice Company is looking for a compassionate and dedicated Hospice RN Case Manager to join our field-based team in Saint Charles, IL. Responsibilities:

Manage a caseload of hospice patients, conducting in-home assessments and developing individualized care plans

Perform skilled nursing visits to monitor patient symptoms, pain management, and overall comfort

Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides to coordinate holistic patient care

Educate and support patients and their families on disease progression, medication management, and comfort care techniques

Document clinical findings, care plans, and visit notes accurately and in compliance with hospice regulations

Ensure compliance with Medicare/Medicaid hospice guidelines and agency policies

Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.

Participates in on-call duties as defined by the on-call policy.

Ensures that arrangements for equipment and other necessary items and services are available.

Attends bi-monthly interdisciplinary group meetings.

Requirements:

Active RN license in the state of Illinois (required)

Minimum 1-2 years of clinical nursing experience; hospice, palliative care, or home health experience preferred

Strong knowledge of pain and symptom management in end-of-life care

Maintains a current CPR certification.

Excellent communication and interpersonal skills with patients, families, and care teams

Ability to work independently in a field-based, self-directed environment

Valid driver's license and reliable transportation for home visits in the Saint Charles, IL area

Proficiency with electronic medical records (EMR) documentation

Empathetic, patient-centered demeanor with a commitment to dignity and compassionate care
