Psychiatric Mental Health Nurse Practitioner (PMHNP)

This position may work out of Connect Health + Wellness Bassett or Ridgeway clinics but will support patients at each of our Primary Care sites.

Position Summary: The Psychiatric Mental Health Nurse Practitioner (PMHNP) delivers compassionate, patient-centered psychiatric care across the lifespan. This role involves collaboration within a multidisciplinary care team to assess, diagnose, and treat mental health conditions, create individualized treatment plans, and support patients in managing their mental wellness.

Key Responsibilities

Patient Care

Perform comprehensive psychiatric assessments, including history, mental status exams, and diagnostic evaluations.

Develop, implement, and revise individualized treatment plans.

Prescribe and manage psychiatric medications in accordance with clinical guidelines and applicable regulations.

Provide psychotherapy (individual, group, or family), as appropriate.

Educate patients and families on diagnoses, treatment options, and preventive strategies.

Interdisciplinary Collaboration

Collaborate with primary care providers, social workers, and other team members to deliver integrated care.

Actively participate in team meetings to review and adjust patient care plans.

Serve as a resource for staff regarding behavioral health concerns.

Documentation

Maintain accurate, thorough, and timely documentation in the electronic health record (EHR) system in accordance with organizational and regulatory standards.

Quality Improvement

Engage in continuous quality improvement efforts to enhance clinical outcomes and service delivery.

Stay informed on current best practices and evolving trends in psychiatric care.

Community Engagement

Support mental health education and outreach efforts to reduce stigma and promote awareness.

Collaborate with community organizations to improve access to behavioral health resources.

Other Duties

Perform additional duties as assigned by the supervisor that are aligned with the mission and scope of this role.

Pay Range: $96,000 to $120,000
