RN MDS Coordinator

About the role

Coordinate MDS assessments and care plans for residents of Evergreen Living Center. This non-union nursing leadership position has a flexible Monday-Friday schedule of either five 8-hour shifts or four 10-hour shifts, includes on-call rotations and one weekend per month.

What you'll do

Coordinate clinical MDS assessments for all Evergreen residents

Ensure department heads complete assigned assessment sections accurately and on time

Create and distribute assessment schedules to appropriate staff

Coordinate the completion, updating, and revision of resident care plans

Participate in resident care plan meetings and document summaries in medical records

Work with CNAs to improve documentation accuracy and provide education and assistance

Participate in nursing leadership and on-call rotations

Work one weekend per month

Assist the facility with additional projects and responsibilities as needed

What you'll bring

Active RN license

Prior MDS experience preferred but not required
