Nurse Practitioner or Physician Assistant

Job Description

We are seeking a compassionate and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our outpatient primary care team. This is an excellent opportunity for a provider who is passionate about delivering high-quality, patient-centered care and improving access to helathcare within the community.

The position is 100% outpatient. You will work as part of a collaborative, multidisciplinary team focused on providing comprehensive primary and preventive care to a diverse patient population.

Key Responsibilities:

Provide comprehensive outpatient primary care to patients across the lifespan, as appropriate to training and experience

Evaluate, diagnose, and manage acute and chronic medical conditions

Perform routine physical examinations and preventive health screenings

Develop and implement individualized treatment plans

Order and interpret appropriate laboratory and diagnostic studies

Prescribe medications and treatments within scope of practice

Educate patients regarding preventive care, chronic disease management, medications, and healthy lifestyle choices

Coordinate care with Physicians, behavioral health professionals, MA staff, specialists and community resources

Maintain accurate and timely documentation in the electronic health record

Support quality improvement initiatives and FQHC clinical performance measures

Provie culturally responsive, compassionate care to underserved and medically vulnerable populations

Qualifications:

Graduate of an accredited Nurse Practitioner or Physician Assistant program

Current or eligible NP or PA licensure in Georgia

Current national board certification

DEA registration or eligibility to obtain

Current BLS certification

Strong clinical judgement, communication, and interpersonal skills

Ability to work effectively within a team-based care environment

Commitment to serving diverse and underserved patient populations

Primary care, Family medicine, internal medicine, or community health experience preferred

New graduates may be considered depending on training and clinical experience
