About the Role
Our client is looking for a licensed Pharmacist to join their home infusion team, providing safe, patient-centered pharmacy care in the alternate care setting. You'll manage the full clinical picture for home infusion patients - from therapy assessment through to dispensing, monitoring, and ongoing care coordination - while ensuring compliance with all applicable regulatory and accrediting standards.
This role reports to the Pharmacy Manager or Director of Pharmacy, depending on branch structure.
What You'll Do
Assess patient suitability for home care and the appropriateness of prescribed therapy
Build and maintain complete patient profiles and pharmacy plans of care
Review labs and perform pharmacokinetic dosing, with appropriate dosing recommendations
Accurately process prescription orders, ensuring proper concentration, compatibility, and storage
Dispense medications and supplies, and program/troubleshoot infusion pumps
Submit electronic claims and manage prior authorizations as needed
Educate and counsel patients and caregivers on therapy, medication use, and infusion methods
Document thoroughly across the full course of therapy, including QA/QC and compounding records
Ensure all clean room and compounding activities meet USP <797> and state/federal standards
Oversee and delegate to pharmacy support staff; help train interns and pharmacy students
Participate in on-call rotation and patient care conferences (including Nutrition Support Rounds)
Step into a Pharmacist in Charge or branch manager role during temporary absences, as needed
What You'll Bring
BS in Pharmacy required (PharmD or MS in Pharmacy a plus)
Active pharmacist license in the state of employment
2+ years in an acute-care setting, or 1+ year in home infusion therapy
Strong background in IV admixture services and IV therapy
Comfort collaborating with physicians, nurses, and the broader care team
Strong organizational, time-management, and problem-solving skills
Solid communication skills, both written and verbal
Working knowledge of MS Office / Outlook
Flexibility with scheduling, including participation in on-call coverage
