Job Description
We are seeking experienced and motivated Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for patients throughout North Carolina. This is an excellent opportunity for providers looking to supplement their income with a flexible, part-time schedule.
Key Responsibilities:
Conduct comprehensive in-home Health Risk Assessments (HRAs)
Identify and document acute and chronic medical conditions
Obtain vital signs and calculate BMI
Review medications and pharmacological therapy
Perform a physical examination
Evaluate patients’ physical, mental, and social health needs
Educate and counsel patients and their families on health conditions and preventative care
Schedule & Flexibility:
Part-time, flexible scheduling
Set your own hours
Visits available 7 days a week
Minimum commitment of 8–10 hours per week
Compensation:
$90 per completed in-home assessment
Mileage reimbursement provided
No-show compensation included
Qualifications:
Active Nurse Practitioner license in the state of North Carolina
Experience with Health Risk Assessments (HRA) preferred
Strong clinical assessment and documentation skills
Ability to work independently and manage schedule efficiently
Reliable transportation for in-home visits
