Nurse Practitioner

As a Nurse Practitioner (NP) or Physician Assistant (PA), you will deliver high-quality preventative care to residents in skilled nursing facilities, combining on-site visits with remote work from home.
About the Role:
As a hybrid Nurse Practitioner, you will work closely with our experienced clinical team to provide comprehensive preventative care, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our program. This strategic integration aims to minimize hospitalizations, elevating patient care standards, boosting CMS ratings, and increasing reimbursement for our facilities.
Responsibilities:
Provide high-quality preventative care to residents in skilled nursing facilities
Conduct on-site visits and remote work from home, supported by a dedicated scribe team for efficient documentation
Build strong relationships with facilities, maintaining a patient-centered approach and being a collaborative team player
Stay up-to-date with industry trends and best practices, attending regular training sessions and conferences
Requirements:
Active NP or PA license (in the state of practice)
1+ year of experience in Geriatric care and documentation review
Excellent communication, collaboration, and teamwork skills
Compensation and Benefits:
$84,000 - $86,000 ANNUALLY
