Program Coordinator (0039-0007)

It is seeking the services of a Program Coordinator to carry out the following: Coordinate and oversee daily client support services and social service programs for elderly and disabled participants at the adult day health care center. Assess participant social service needs and coordinate appropriate community resources, wellness programs, transportation assistance, and family support services. Develop and implement culturally appropriate client engagement activities, educational workshops, and wellness support programs for participants and caregivers. Maintain communication with clients’ families, caregivers, healthcare providers, and community organizations regarding participant services and program participation. Assist management in program planning, operational coordination, scheduling, staff support, and quality improvement initiatives. Prepare reports, maintain participant records, and ensure compliance with applicable program policies and administrative procedures. Coordinate intake procedures, participant orientation, and service documentation. Assist with administrative operations, budgeting support, vendor coordination, and office management functions related to program operations. Participate in development of community partnerships and social service referral networks.

Education and Experience Requirements

Bachelor's degree in Social Work/Welfare related
