Partnering with Medicare Advantage plans, the group is dedicated to improving outcomes through a patient first, team based approach and is seeking Primary Care Internal Medicine Physicians to join their innovative care model near Montgomery, AL.

This Position Offers:

Reduced patient panel size to 400 or fewer patients, allowing for more meaningful patient interactions

Value based, outcomes driven care model with no fee for service pressure

Collaborative, team based environment including physicians, APPs, pharmacists, behavioral health, and care coordinators

Strong administrative and clinical support, including coders and referral specialists

Minimal call responsibilities
