Work Environment

The work setting at ProActive Therapy Centre is an outpatient office located in the vibrant city of Sacramento. Key Responsibilities

Conduct thorough evaluations by reviewing patients' medical histories, performing specific assessments, and documenting findings clearly to create effective treatment plans.

Engage and educate patients and their families about evaluation results and collaboratively set realistic treatment goals to foster engagement and adherence to therapy.

Implement treatment protocols tailored to each patient based on their evaluations, progress, and changes in their health status, ensuring adherence to evolving best practices in physical therapy.

Collaborate with other healthcare professionals as needed to provide comprehensive care and promptly document any changes in patient conditions that necessitate adjustments to treatment plans.

Conduct appropriate discharge planning, ensuring patients understand follow-up care while maintaining accurate and timely documentation of all patient interactions.

Manage your schedule efficiently to meet productivity expectations while being punctual for meetings and contributing to outreach activities that promote our services in the community.

Participate in departmental initiatives to achieve established goals, adhering to regulatory compliance and accreditation standards.

Pursue continuous professional development by engaging in educational opportunities and mentoring students as a clinical instructor.

Enhance the patient experience by demonstrating adaptability and flexibility in treatment approaches, helping to eliminate barriers to access and improving care access across the department.

Patient Population / Client Focus

The physical therapist will work primarily with outpatient clients, including individuals recovering from injuries, surgeries, and chronic pain conditions. Our diverse patient population ranges from athletes seeking rehabilitation to elderly clients requiring mobility assistance, allowing for a dynamic practice that focuses on personalized care and functional improvement.

Qualifications

A Bachelor’s degree in Physical Therapy from an accredited institution.

A valid state license required to practice as a Physical Therapist in California.

Current American Heart Association BLS for Healthcare Providers CPR certification.

While prior work experience is not required, candidates with a passion for patient care and a willingness to grow professionally are strongly encouraged to apply.

Preferred Experience

Experience in an outpatient setting is beneficial but not mandatory.

Familiarity with electronic health record (EHR) systems is a plus.

Compensation and Benefits
