At Reliant Health Center, we believe in fostering a supportive online community that values professional development and teamwork.Key Responsibilities\n As a Licensed Professional Counselor with Reliant Health Center, you will be responsible for the following:\n\n Conduct regular and thorough assessments to understand the needs of your patients and tailor treatment plans accordingly.\n Communicate effectively with supervisors, direct care staff, family members, and guardians about patient progress and treatment updates.\n Manage a schedule of appointments, home visits, and observations, coordinating efforts to ensure effective monitoring of treatment outcomes.\n Conduct home visits to perform in-depth observations, chart reviews, and interviews to assess individual needs and progress.\n Implement and review program plans, ensuring that each element is enacted effectively and aligns with established treatment goals.\n Participate actively in interdisciplinary team meetings to share insights and collaborate on treatment strategies.\n Maintain clear and comprehensive monthly and quarterly progress notes that are aligned with patient treatment plans.\n Organize and lead training sessions to ensure staff competence in program execution and documentation standards.\n Provide ongoing follow-ups, including interviews and additional home visits, to assess continued support requirements for patients.\n Keep precise and organized records to facilitate easy access to patient information for all stakeholders involved.\n Act as a liaison with the community and mental health organizations to foster support for local development initiatives.\n Assist in the creation of training materials that enhance the effectiveness of programs designed for the improvement of client services.\nPatient Population / Client Focus\n Your work will encompass a diverse client base, including both adults and youth dealing with various mental health challenges.

You will have the opportunity to work with clients experiencing trauma, substance use issues, and relational conflicts, utilizing evidence-based practices that make a profound difference in their lives.Qualifications\n The ideal candidate will possess:\n\n A Master’s or Doctoral degree in Counseling, Psychology, or a related field.\n A valid and unrestricted license as a Professional Counselor (LPC) in the state of Oregon.\n A minimum of two years of counseling experience, preferably within a clinical setting.\n A robust understanding of legal and ethical standards related to mental health counseling.\n Exceptional interpersonal and communication skills to foster rapport with a wide array of clientele.\n A demonstrated ability to work collaboratively within a healthcare team.\nPreferred Experience\n While not mandatory, candidates with specialization in family therapy, couples therapy, or trauma support will be given preference.

Familiarity with EMR software and technologies relevant to telehealth practices will also be an advantage.Compensation and Benefits\n Reliant Health Center offers a competitive salary ranging from USD 90,000 to 100,000 annually, which may vary based on caseload, availability, and experience.
