Our experienced clinical team is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities.

About the Role:

We are seeking a highly skilled and dedicated NP or PA to join our team of healthcare professionals. As a hybrid role, you will combine on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Our ideal candidate will have a strong background in Geriatrics or Internal Medicine and excellent communication, collaboration, and teamwork skills.

Responsibilities:

Deliver high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work

Conduct patient assessments and develop treatment plans in collaboration with the healthcare team

Provide chronic care management and remote patient monitoring to patients with chronic conditions

Collaborate with the healthcare team to ensure seamless patient care and minimize hospitalizations

Participate in regular team meetings and contribute to quality improvement initiatives

Requirements:

Active NP or PA license in the state of practice

1+ year of experience in Geriatric care and documentation review

Excellent communication, collaboration, and teamwork skills

Masters' degree in Nursing or Physician Assistant Studies
