Job Summary
This PRN Speech-Language Pathologist position offers flexible scheduling with the option to provide services either in person or through telehealth, based on patient needs, therapist availability, and scheduling.
Key Responsibilities
Evaluate patients to identify speech, language, communication, cognitive, and swallowing needs.
Develop individualized treatment plans based on patient needs and goals.
Provide appropriate speech and language therapy interventions.
Monitor patient progress and adjust treatment plans as needed.
Maintain accurate and timely patient documentation and treatment records.
Communicate effectively with patients, families, caregivers, and other healthcare professionals.
Collaborate with other members of the healthcare team to support patient care.
Provide compassionate and professional patient-centered care.
Follow applicable clinical, safety, and documentation standards.
Qualifications
Must hold a valid Speech-Language Pathologist (SLP) license in the State of Illinois.
Proven experience working as a Speech-Language Pathologist.
Strong communication and interpersonal skills.
Compassionate, professional, and patient-focused.
Ability to work independently and as part of a team.
Strong organizational and documentation skills.
