POSITION SUMMARY

The Registered Nurse Supervisor is responsible for the overall leadership of clinical nursing services, and assumes leadership of nursing personnel (RN, LPN, at designated site(s). They will clinically supervise Registered Nurses, License-Practical Nurses, as assigned, and act as liaison with accredited Nursing programs. In collaboration with the Senior Manager of Nurse and Quality Improvement & Chief Clinical Operations Officer, the Nurse Supervisor will play a vital role in the preparation for on-site clinical and operational audits, as well as the development and monitoring of quality improvement projects and Uniform Data System (UDS) and Health Effectiveness Data Information Set (HEDIS) quality metrics. The Registered Nurse Supervisor is directly responsible for the infection prevention and control program and all activities.

Qualifications

ESSENTIAL SKILLS AND EXPERIENCE:

Bachelor’s Degree from accredited university or foreign equivalent.

Minimum of two (2) years increasingly responsible nursing management, experience preferably in outpatient setting.

Minimum of five (5) years experience in direct patient care, preferably in an ambulatory care setting;

Graduation from an accredited nursing program.

Certification in Infection Prevention Control (CIC) or ability to acquire one within the first year of employment by meeting the CIC qualifications of:

At least 1 year of full time experience in infection prevention control OR two years of part time infection prevention control experience OR completed 3,000 hours of infection prevention control experience earned in the previous three years

Licensed with AZ State Board of nursing; license in good standing

Demonstrated understanding of clinical methods and techniques used in record-keeping systems.

Advanced computer efficiency in Microsoft Office suite (Word, Excel, PowerPoint, Outlook) and experience in Patient Management System

Meet the organization immunization requirement

Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills;

Demonstrated ability to work effectively with men and women of diverse races, ethnicities, ages and sexual orientations in a multicultural environment.

Valid Arizona driver’s license

Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations)

Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations

POSITION RESPONSIBILITIES/ESSENTIAL:

Staff Development and Management

Assist and support in all human resources functions for nursing and ancillary service including hiring, firing, and disciplinary action; maintains appropriate documentation and ensures compliance with all company policies and requirements.

Ensures that staffing levels and coverage are adequate to meet patient care and regulatory requirements.

Assumes managerial duties for performance evaluations and maintains systems to provide regular feedback and evaluation for employees.

Enhance a positive environment at all health centers in collaboration with key stakeholders.

Assist the Senior Manager of Nursing and Quality improvement & Nurse Educator in developing and implementing staff training and development programs to meet continuing education and contractual requirements and to further knowledge of medical care and working in team environment.

Program Planning and Management

Assist the Senior Manager of Nursing and Quality Improvement to provide overall direction and guidance for nursing & health programs in health centers.

Works closely with the Senior Manager of Nursing and Quality Improvement and other organizational leaders as appropriate, to ensure that nursing program and ancillary services meet patient care and regulatory requirements.

Oversees and controls supplies that directly impact patient care (e.g., ordering and charging of stock medications, medical supplies and patient care equipment and/or supplies).

Quality Management

Assist the Senior Manager of Nursing and Quality Improvement to provide Quality Management direction and oversight for nursing and ancillary activities in all Adelante Health Centers as appropriate.

Actively participates in Quality Management Committee (QMC) and ensures that regular monitors are conducted and reported on.

Leads and/or participates in various special projects teams to address areas of concern.

Enhance Service Excellence throughout the health centers.

Enhance Care Delivery Model initiatives by being a role model.

Clinical Compliance

Assure that nursing and ancillary staff practice within their individual scopes of practice.

Participate in Nursing Unit Level P&P Committee maintaining current P&P for nursing and lab programs and ensures that they comply with all regulatory and contract requirements.

Participate in or conducts regular meetings with staff to ensure full understanding and buy-in for all Policy & Procedure (P&P).

Participate as needed in all program, licensure and other external audits and in the preparation of Corrective Action Plans (CAP).

Infection Prevention and Control

Direct responsibility for Adelante’s infection prevention and control program and any activities

Work closely and collaboratively with leaders that are associate level interaction prevention and control partners

Performs triage calls using approved clinical decision support tools for the adult primary care, OB, and Peds populations.

Participates in team meetings, designated “Specialty Team” meetings, and quality and process improvement initiatives.

Helps facilitate health and disease patient education (including adults, peds, and OB populations) sessions.

Coordinates continuity of patient care with external healthcare organizations and facilities such as referrals to outside specialties and ongoing two-way communication between providers and care teams.

Develop relationships with external healthcare organizations and facilities in efforts to find additional resources.

Coordinate outreach efforts to high-risk patients and hospital discharge and admissions.

Collaborate with health plans, hospitals, and skilled nursing facilities for transitional needs.

Conduct comprehensive, preventative screening for patients and/or assists all support staff in daily patient interactions as needed.

Completion of Nurse Visits including but not limited to POCT items (flu, strep, UA, HCG testing)

Helps evaluate clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.

Manages insulin titration for the diabetic population, which can include outbound calls to follow up.

Excellent customer service and communication skills

Performs assessment and delivers patient-specific education plan.
