1. Licensed Clinical Social Workers (LCSW) – 2 Positions

Responsibilities:

Facilitate group sessions

Provide behavioral support and care coordination

Assist clients with resource navigation

Document sessions and client progress

Work collaboratively with the Director of Behavioral Services

Requirements:

Active LCSW license

Familiarity with CAQH

Must have an NPI or obtain one within 2 weeks of employment

Strong communication and documentation skills

Background check required

2. Director of Behavioral Services – 1 Position

Responsibilities:

Oversee behavioral support programs

Supervise LCSWs and support staff

Ensure compliance with center standards

Develop group session curriculum

Coordinate with leadership on program expansion

Requirements:

LCSW preferred

Experience in behavioral health or senior services

Familiarity with CAQH and NPI requirements

Leadership experience

Background check required

3. Caregivers / Senior Support Specialists – 3 Positions

Responsibilities:

Provide companionship and daily living support

Assist with errands, transportation, and appointments

Support clients in homes or care facilities

Maintain accurate visit notes

Requirements:

Valid driver’s license

Reliable vehicle

Full time availability

Professional, patient, and dependable
