Coach

Mosaic Life Care is seeking a Job Coach to support individuals in healthcare and social assistance settings in building job readiness and employment success.

The Job Coach will assess strengths, develop individualized employment plans, teach work skills, and provide on-the-job coaching and feedback.

Responsibilities include coordinating with clinical teams, community partners, and employers, tracking progress and outcomes, and maintaining accurate documentation.
