Licensed Practical Nurse (LPN) - Barron, WI

Join the Heritage Lakeside team at our Rehabilitation and Nursing Center in Barron, Wisconsin, where our patient-centered care enables our skilled nursing team to develop personalized care plans for each resident's physical, spiritual, social, and emotional well-being. As a Licensed Practical Nurse (LPN), you will play a vital role in delivering high-quality care to our residents.

Job Summary:

We are seeking a qualified, dedicated, and compassionate LPN to join our team. As an LPN at Heritage Lakeside, you will monitor, plan, and implement the nursing care of residents under the direction of a Registered Nurse (RN). You will ensure continuity of care by providing direct care and supervising the care given by Certified Nursing Assistants (CNAs) and support staff members. You will also participate in resident and family education and serve as a resident advocate, focusing on our facility's mission.

Responsibilities:

• Monitor, plan, and implement the nursing care of residents within the facility under the direction of a RN

• Ensure continuity of care by providing direct care and supervising the care given by CNAs and support staff members

• Participate in resident and family education

• Serve as a resident advocate with a focus on the facility's mission

Qualifications:

• Current/valid Wisconsin LPN license/registration

• Minimum of 0 years of experience (entry-level candidates encouraged to apply)

• Ability to work collaboratively as part of a team
