About the Role:

We are seeking an experienced Nurse Practitioner or Physician Assistant with a background in Geriatrics or Internal Medicine to join our team. You will be responsible for:

Providing patient-centered care to residents in skilled nursing facilities

Conducting on-site visits and remote work from home

Collaborating with our dedicated scribe team for efficient documentation

Building strong relationships with facility staff and maintaining a patient-centered approach

Being a collaborative team player and contributing to our positive and supportive clinical environment

Requirements:

Active NP or PA license (in the state of practice)

1+ year of experience in:

Geriatric care

Documentation review

Excellent communication, collaboration, and teamwork skills
