Fox Subacute South Philadelphia is a 53-bed skilled nursing and subacute facility specializing in the care of clinically complex respiratory and ventilator-dependent residents. Our environment offers the acuity level of a hospital step-down unit while allowing nurses to build long-term relationships with residents and deliver truly personalized care.

Key Responsibilities
Assess, plan, implement, evaluate, and modify resident care plans
Administer medications and treatments accurately and safely
Respond to and manage emergency situations professionally and calmly
Document care and clinical changes using PointClickCare
Collaborate with the interdisciplinary healthcare team
Deliver compassionate, individualized care aligned with resident needs
Qualifications & Requirements
Active Pennsylvania LPN license (required)
Current BLS/CPR certification
Ability to work 12-hour shifts, including nights, weekends, and holidays as needed
Strong communication and critical thinking skills
Ability to remain calm during emergencies and high-acuity situations
Ability to perform physical job duties, including lifting up to 50 pounds
English proficiency preferred
Ability to commute or relocate to Philadelphia, PA 19145
