Pay: Starting at $20.00–$22.00 per hour (based on experience) (Adjust if needed.)

Schedule: Part-Time – 20 hours per week

A Place Called Home LLC is a Minnesota 245D licensed provider dedicated to empowering individuals with disabilities and behavioral health needs to live independently in their communities. We are currently seeking a compassionate, dependable, and motivated Individualized Home Supports (IHS) Practitioner to provide person-centered support to individuals in the Prior Lake and Belle Plaine areas.

Job Responsibilities

As an IHS Practitioner, you will provide individualized support based on each person’s unique goals and support plan. Responsibilities may include:

Assisting individuals with developing and maintaining independent living skills.

Supporting community integration and participation in meaningful activities.

Assisting with meal planning, grocery shopping, and meal preparation.

Providing homemaking support, including light housekeeping and laundry.

Assisting with scheduling and attending appointments as needed.

Supporting medication reminders (when appropriate).

Encouraging skill development, self-advocacy, and independence.

Completing required documentation and EVV accurately and on time.

Communicating regularly with supervisors and members of the individual’s support team.

Qualifications

Required

High school diploma or GED.

Valid driver’s license and reliable transportation.

Current automobile insurance.

Ability to pass a DHS background study.

Strong communication and organizational skills.

Ability to work independently while maintaining professionalism.

Preferred

Experience working with individuals with disabilities, mental health needs, or older adults.

Experience providing HCBS or 245D services.

CPR/First Aid certification (or willingness to obtain).
