Base Salary: $100,000 – $150,000 per year

Position Summary
We are seeking a compassionate and experienced RN Care Manager to join our healthcare team across multiple locations in California. The RN Care Manager will oversee care coordination, lead interdisciplinary teams, and ensure high-quality, patient-centered care while supporting clinical excellence and operational goals.
Key Responsibilities
Coordinate and manage patient care plans across the continuum of care.
Lead and mentor nursing staff while promoting quality improvement initiatives.
Collaborate with physicians, case managers, social workers, and other healthcare professionals.
Monitor patient outcomes and ensure compliance with regulatory and accreditation standards.
Assess patient needs and develop individualized care plans.
Educate patients and families regarding treatment plans and available resources.
Participate in quality assurance, performance improvement, and care management programs.
Maintain accurate clinical documentation and reporting.
Required Qualifications
Active and unrestricted California Registered Nurse (RN) License.
Minimum 2 years of Registered Nurse (RN) clinical experience.
Minimum 2 years of leadership or supervisory experience in a healthcare setting.
Strong care coordination, communication, and problem-solving skills.
Ability to work collaboratively in a multidisciplinary team environment.
Current BLS certification (ACLS preferred, depending on department).
Preferred Qualifications
Bachelor's degree in Nursing (BSN) preferred; MSN is a plus.
Previous experience in case management, care management, utilization management, or population health.
Certification in Care Management (CCM or ACM) is preferred.
