Graduate Practical Nurse (GPN)

Rate: $25.54/hour

Shift: All Shifts

Autumn View Health Care Facility is looking to add a Graduate Practical Nurse (GPN) to its nursing team. In this role, you'll work under the supervision of a Registered Nurse to provide compassionate nursing care and support to residents while gaining valuable hands-on experience in a skilled nursing environment. RESPONSIBILITIES:

Provide nursing care under the supervision of a Registered Nurse, including medication administration and treatments

Assist with developing, implementing, and updating resident care plans

Monitor resident conditions and promptly report changes to the supervising nurse

Provide personal care assistance, including hygiene, mobility, and feeding support as needed

Accurately document resident care, treatments, and observations

Maintain a safe, comfortable, and resident-centered environment

Collaborate with nursing staff and interdisciplinary team members to support quality resident care

Follow facility policies, healthcare regulations, infection control practices, and safety procedures

Participate in team meetings and ongoing education opportunities

Perform additional duties as assigned

REQUIREMENTS:

High School Diploma or equivalent preferred

Graduate Practical Nurse eligible for licensure in New York State

Strong written and verbal communication skills

Ability to follow written and verbal instructions

Compassionate, resident-focused approach to care

Ability to work effectively with residents, families, physicians, and team members

Strong organizational and documentation skills

Ability to perform frequent physical activity and assist with resident care needs
