heat prior to commencing work on items requiring such, verified origin of materials being used and consumables used during

fabrication, performed pre coating prep visual inspections of parts to receive surface prep and or coatings, then final visual

following blasting, and or coating application and finally placing the Port Authority stamp on each piece/sub assembly and

generating and completing the shipping documents sent out with each item or piece. Reported directly to the NY/NJ Port Authority

AMEC QA CWI/UT MT LII Texas Cross Trans Line, Tulsa OK, Dallas Ft. Worth Huntington TX,

02/12 to 06/12. Performed QA UT shearwave inspections of 100% of the welds inspected by the various

QC personnel at the 4 locations. Inspected metal power pole CJP welds identified to contain known defects from the fabrication

company that welded these connections initially. Identified numerous layers of defects in each of the approximately 500 pieces

examined, plotted and marked these defects for repair, then verified each defect had been removed and re welded to meet the

inspection criteria. Worked closely with QC and production welding supervisors and welders to try to minimize multiple repairs on

any one piece. Performed UT inspections constantly during the 10 hour days identifying literally thousands of class A, B and C

defects. Documentation and reporting so many indications per piece along with at times multiple repairs prior to acceptance was

Performed CWI and ICC High Strength bolting and receipt inspections as required to complete the project. Work plan review,

procedure compliance observations, procedure and document review as required to support the craft. Performed process piping

inspections as requested by LES engineering in the 1001 copper and stainless Nitrogen system installation. Both projects

Provided QA/QC, CWI, NDT, document control, plans and wps adherence supervision duties for a D1.5 shop providing mainly sign

bridge and AISC type structural steel for the DOT’s for many states, often working under the direct scrutiny of Level III inspectors

for those agencies. Many hundreds of CPJ welds were inspected using D1.5, as like anyone using that code there were also

numerous repairs to be re worked and re inspected. Was able to provide the QA inspectors clean defect free work consistently.

Document control for QA manual revisions/audit’s, MTR’s, plan and acceptance criteria compliance and documentation, C.O.C’s

Provided both shop and field erection special inspections, required NDT inspections and third party plan verification inspections of

structural steel as an AWS CWI, ICC SS & Welding Special Inspector, WABO Building Official, OBOA Building Official, and City of

Portland SS Special Inspector. Took and passed the FEMA T operators exam twice and passed both times. Performed the

majority of the SMRF and FEME 353 CPJ special connections for most of the time at Carlson. Performed mainly shop and field

Performed NDT inspections as a level II Radiographer, UT Shearwave, Magnetic Particle, Liquid Penetrant, and Mechanical Level

visual inspector at industrial settings like Nuclear power plants, Pulp & Paper, refineries, petroleum and chemical plants, pipelines,

pressure vessels, storage vessels and related process piping, aero space, hydro electric and other power producing facilities both

on and off shore all over North America. Many standards including ASME, ANSI, Mil Standards, API 510, 653 and 570, API 1104

and many others dealing with too many different materials and shapes to list. Was a member of the team that initiated the API 570

program in Kingsport TN at Tennessee Eastman chemical plant. Extensive UT baseline as well as in process evaluations were

initiated using UT. Many various types of materials are used requiring an extensive box of calibration equipment. We did thickness,

erosion/corrosion, and component fatigue evaluations starting a whole new way of inspection at the facility. Was a part of the team

who developed the thinwall tube inspection by shearwave of new boiler tube welds. Our team, along with oversight RT teams

Performed much the same type of NDT inspections at the same type of industrial settings across the US as a Level II Ultrasound,

Radiographer, Magnetic Particle and Liquid Penetrant inspector. Extensive travel and 12 hr shifts 7 days a week until

outage/project completion 300 days a year. Thousands of hours doing UT thickness surveys in 100’s of plants. Hundreds of hours

doing B Scan UY of reactors and other heavily insulated items, hundreds of hours verifying material thickness at steel production

plants, IGSCC inspection of steam piping for power plants sizing known defects as part of a corrosion analisis project for a multi
