OBJECTIVE

I am looking for a position which will allow me to apply my diverse background of experience in all areas of nursing, including management and hospice, in a facility with extremely high standards of care for the elderly and dying. I am presently working through a temporary agency in a supervisory capacity.

QUALIFICATIONS

Since graduating as a RN, I have held positions from CNA to DON in Long-term Care. I also have extensive experience in Home Care, Hospice Nursing, Ventilator Care and CBRF/ALF management. I am intent on finding a facility that has the patient’s interest as their utmost concern and provide dignity and compassion in the care they provide.

1981-1983 Associate Degree- Nursing Milwaukee Area Technical College

1996- 2000 Completed my BSN but was unable to attend the graduation in NY

EMPLOYMENT

3/09- 9/10 –Dove Healthcare and rehab- PM Supervisor, MDS coordinator(for a short period). Assessment of change of condition, in-servicing staff on new owners and hospice protocol teaching staff the computer charting system, working with the Evercare APNP, supervision of staff, and duties as assigned. Assisted in relocation of residents when one facility closed.

8/06-7/08 RN Day Supervisor/ Unit Manager/ Weekend House Supervisor- Wisconsin Lutheran Care and Rehab While working at this facility, I was moved, due to numerous employee reassignments to numerous positions in nursing. I was in charge of a Medicare Unit and the house for an extended period of time. All the assessments, staffings, IV’s, MDS completions, employee evaluations were done through me.

1/05-7/06 RN Manager Bel Aire Health Care Center- 24 hour accountability for 48 bed unit. The unit has Medicare, geropsych and rehab patients. Responsibilities included MDS coordination, assessing any change of condition, psychotropic medication changes and staff supervision and discipline. As the only RN working the floor, I am also responsible for the other three units in the building and provide backup for the LPN managers. I was also instrumental in developing policy and procedures that were required for our Plan of Correction for the State.

10/04-1/05 Director of Nursing Premier Care Center-Mitchell Manor This position was taken with the idea that the facility would be willing to upgrade the facility to provide excellent care for the residents. I did not know that the facility was devoted to not changing the policies to ensure compliancy with State and Fed regulations. While I had this position I was responsible for staffing, admissions, Medicare compliance, hiring, disciplinary actions and retention along with redoing any missing policies and procedures to bring facility into compliance. I was also responsible for improving quality of care for residents and handling family concerns as well as meeting with sales reps and physicians. All ancillary departments brought problems to me that needed resolution. Facility had both SNF and CBRF divisions and I was responsible for the updates that were required to get CBRF in compliance. Also was responsible for any medical emergencies in the attached assisted living facility.

2003-2004 CM and weekend on-call RN – Vitas Hospice

Was requested to rejoin Vitas Hospice to do the weekend on-call and handle any of the emergencies that arose on the weekends. Was responsible for obtaining the Legals and IPOC’s on all admissions, since I was the only RN on during the weekend. Was also responsible for all emergencies that required a RN to handle (i.e. IV's CADD pumps and any needed medication changes a/o assessments. Also carried a complete case load as a Case Manager of 18-32 patients. Was responsible for recertification and discharging pt’s for extended prognosis if they were not appropriate. Was supervising between 8-12 CHHAs during this time.

2002-2003 Clinical Manager-Horizon HH and CM Covenant Hospice

Worked at Horizon as the Clinical Manager for the NE team and was responsible for making sure all areas of care were in compliance. Inserviced and instructed staff with any technical needs. Completed Oasis authorization paperwork for Medicare. Supervised 12 nurses and 9 HHA . Recruited by DON at Horizon to move to Covenant Hospice and become a case manager to handle weekend emergencies

1998-2002 Independent Practice Respiratory Care Specialist: Care of ventilator dependent patients in their homes on a 1:1 basis. Responsible

for total care of the patient. All assessments, MD interaction and proving a home

-like environment, which included cooking and cleaning, were my responsibility. Problem solving, my forte.

1997-1998 Ruth Hospice- case manager

Residential Hospice care for 7-14 patients. Assisted in developing hospice policies and protocol for the new facility. Pain medication titration, personal care of the dying patient and psychological care of the families were responsibilities. Also worked at St Mary's Hospice during this time. Worked with the acute care hospice patients with difficult symptom management and palliative needs. Am very adept with central lines from this experience.

1995-1997 Case manager- Vitas Home Care and Hospice:

Caseload of 15-17 terminal patients in LTF’s. Adjusted pain medications, made daily visits, monitored and managed symptoms. On-call services for after hour emergencies, especially IV med administration

1993-1995 ADON-St Joseph's Home for the Aged:

Functioned as the Day Supervisor/ ADON, occasionally DON /Administrator on-call. Did statistical work, infection control, staffing for all floors and DISCUS evaluations for the facility. This facility was both acute care and assisted living. Also, integrated services between all disciplines and ancillary depts. Also, was responsible for disciplinary actions dealing with staff.

1991-1993 Unit Manager- Medicare Unit North Shore health Care Center:

In charge of a 52-bed Medicare Unit Staff education, infection control, admission, change of condition, and house supervision of 264 beds. Again, was involved in

fast track correction that led to two years of deficiency free surveys.

1983-1986 Staff Nurse promoted to Unit Manager- Mequon Care Center:

Charge of a 36 bed Alzheimer's Unit, Responsible for admissions, change of condition, Psychiatric review and medication referral to physicians. Developed a wandering patient protocol and identification system of these patients.

CONTINUING EDUCATION

Certified in Geriatric Nursing, Hospice, Aromatherapy, Reflexology, CPR, State certification in ventilator care, Acupressure and Herbal Medicine. Previously attended courses in pain management, respiratory, wound care, psychotropic drugs, Medicare/Medicaid reimbursement, IV therapy and emergency medicine assessment and treatment. Have also completed extensive pain and palliative treatment courses within the past year.
