I am writing you this brief letter as an overview of my experience in nursing.
I have been in nursing for approximately 32 years. I started out as a LPN and later obtained my BSN. During this time I have worked in just about every setting for nursing.
Most of my nursing experience has been in the ED, although I have also worked in a doctor’s office, med-surg floor, head injury rehab, and as a House Supervisor. My career has taken a turn and for the past few years I have been in case management. I have enjoyed each area and learned a great deal about the nursing process and the changes that have come about over the years, but UR and Case Management has opened my eyes to what happens to the patient after he/she leaves the hospital and what qualifies as an inpatient or observation status according to Interqual and Milliman. Also what is needed by the patient after discharge, such as Home Health, DME, Social Services, etc.

Objective
My objective is to find a position where I can continue to expand my role in case management and/or utilization review. I began my career at the age of 19 as a LPN and obtained my BSN in 1998. I have worked in all areas of the hospital, although most of my experience has been in ED and was House Supervisor for over 5 years. In that role I quickly learned the meaning of multitasking and the art of working with different departments in the hospital. The importance of working with the patients and their families is very high on my priority. Acting as a go between for the patients and the doctors is a skill that is important in any area of nursing. I have reached a point in my career where I am no longer able to do bedside care for 12 hours, but have found that Case Management allows me to still be involved in patient care and work with staff and doctors. I find this newer aspect of my career challenging and fulfilling. I have learned the importance of getting the patient through the hospital stay as quickly and as safely as possible. I have also learned to think beyond the hospital setting and consider what the
patient’s needs will be once discharged from the hospital and what insurances, Medicare, and TennCare will pay for and what types of services can be obtained for free or at a reduced price.
Qualifications
I have been in nursing for 32 years, with experience in all areas of the hospital, except OR. I have management experience as House Supervisor and Case Management. I believe my clinical experience has made me very capable to decide if an admission needs to be an observation or full admit. I have learned what different insurance companies expect and want to know in clinical updates. As well as what is required for each diagnosis is needed to qualify as a full admit and which diagnoses are never full admissions. Even as a Case Manager, I feel I still can offer younger nurses the knowledge I have obtained over the years in nursing and to think about what the patients needs will be in the future.

Case Management and Utilization Review
Since working in this role, I have learned how to code diagnosis for insurance notification/pre-certification to obtain the best payment rate. Update the patients progress of lack of progress and barriers to discharge. Work with doctors and staff to provide the best, safest, and most cost effective manner. Coordinating tests in different departments to minimize delays and shorten the length of stay for patients. Help develop and coordinate discharge plans to fit each patient’s needs. Nursing Homes, SNF units, home health, PT/OT, follow up appointments. I have worked with the Health Department with TB patients.

Middle Tennessee Medical Center 2002-2007
My duties included: Staffing in the nursing units, handling patient complaints, employee disputes, assisting with critical patients. (During codes, running codes under the ED MD’s orders until the patient’s PCP arrived.) Checking on patients the nursing staff was concerned that their condition had worsened. Assisted beds, transferring patients out and accepting patients from other facilities.
Settling disputes between different departments in the hospital, nursing, lab, Radiology, EVS, and engineering. (Which in itself was a full time job.) Helped control overtime and the use of agency personnel. Counseling staff as needed. Prepared daily reports on census for each floor, surgeries expected and where they might be placed.
In general represented Administration Friday thru Sunday nights and troubleshot all problems that could occur during the weekend.
Middle Tennessee Medical Center 2000-2002
RN staff nurse ED
Charge nurse, triage, cared for all types of patients in all age groups. Critical and non-critical patients, codes and adults and pediatric patients. Assisting physicians with procedures for simple pelvic exams to more complex procedures. All while keeping track of what was going on in the rest of the ED.

After obtaining my BSN I was trained as a charge nurse and triage nurse. Assisted with all types of traumas. Including cracking chests and internal cardiac massage.
