Position Title: Clinical Generalist Pharmacist – Inpatient [February 2011 – Present, 60% FTE]
Clinical generalist covering a multitude of patient populations in a decentralized setting including specialities of critical care, hepatology, infectious disease, and general medicine. Primary responsibilities on a daily basis include order verification, order entry, pharmacokinetic calculations, renal dosing adjustments, and patient monitoring. Schedule flexibility has allowed me to be cross-trained on all generalist positions and surgical intensive care unit satellite. Participation in Safety Solutions committee with focus of patient safety on current policies/procedures, and other various guidelines. Also involved in Pharmacy and Therapeutics committee creating formulary substitution guideline and general reference guideline.
Position Title: Specialty Practice Pharmacist – Outpatient [November 2011 – Present, 40% FTE]
Specialty practice pharmacist providing ambulatory care services including both anticoagulation management and anti-arrhythmic medication monitoring.
