1/2011-5/2011 Lexington Healthcare Director of Nursing
Maintained a large nursing staff (100+) and optimal capacity of residents/patients (245).IDPH survey after all corrections was a desk review! Maintained weekly IDT meetings regarding falls, weights, wounds, infections, reportables, etc. to ensure IDPH compliance and maintain patient safety. Subacute was the main focus of this tenure and our census was slowly developing while maintaining optimal long term and memory care. Accountability of staff was also a main focus.
7/2010-1/2011 Midway Neurological Director of Nursing
Maintained a large nursing staff as well as 300+ residents/patients. Including sub acute, long term care, psychiatric and memory care. Maintained good standing with staff, residents, families and physicians,. Worked very closely with all departments with daily IDT meetings regarding falls, wounds and any other important issues. Maintained weekly IDT meetings with all departments to ensure weights, wounds, falls, infections, reportables, etc. Charts addressed regarding care plans, new orders and documentation to ensure IDPH compliance and patient safety. With proper teaching and in servicing I have maintained a good standing with IDPH(5 complaint surveys with a possible 25 tags and 0 findings)Annual survey with 5 tags (nursing) all desk reviews. Experienced HUD inspection during this tenure. This building was a welcome challenge and I am proud of my performance!
2009-/2010 Care Centers Assistant Direction of Nursing
During this tenure I obtained knowledge of the psychiatric patient/resident and memory care dedicated unit. Continuing to add knowledge of IDPH regulation and compliance. Learned necessary tools to operate 240+ facility. Obtained knowledge of scheduling, accountability of time punches/absenteeism and hiring process. Professionalism and accountability maintained while obtaining optimal capacity.
11/07-11/08 Evergreen Healthcare Center Assistant Director of Nursing
Duties include accountability, professionalism and compliance with IDPH and JACHO regulation while maintaining optimal capacity. Trained with DON to further my career to advance to DON when prepared. Learned necessary steps to maintain a large facility including sub acute, long term and memory care. Maintained all staffing issues. Learned corporate compliance and weekly reports.
2005-2007 Care Centers Floor nurse to ADON
Training IDPH regulation and compliance. Learned necessary tools to operate a 200+ bed facility dual certified. Professionalism and accountability maintained while maintaining optimal capacity. Assisted with a decertification acquired from previous owner. This taught me a tremendous insight of the IDPH process.

This tenure taught me acute assessment and all care of the oncology patient.
