LVN duties and Responsibilities: Assists in the assessment of patient needs and health plan by taking vital signs, blood pressure, and patient history, medication listing, assessing visual acuity and determining chief complaint.

On call 24 hours for patient or care givers concerns regarding care.

Administer prescribed oral medications, intramuscular or subcutaneous injections. Complete all necessary documentation in the patient’s medical record in accordance with practice’s EMR procedures.

Prepare equipment and assist physicians with medical treatments, exams, and medical procedures.

Arrange for specialized consultations and appointments for tests or diagnostic procedures as ordered by the physicians

Verify with appropriate insurance personnel to pre-certify surgeries or tests ordered by the physician or other health care provider, requiring pre-certification by insurance

Provide skilled nursing care to patients /clients in a home setting. Assess home health clients to identify the physical, psycho-social, and environmental needs.

Implement, develop, and document the plan of care to ensure patient quality and continuity of care.

Provide health care education to client families and or caregiver. Provide education for effective communication to clients /family, team members, and other health care professionals.

Assist with admission processes regarding documentation, new medications, diets, or treatments for clients, families, and care givers as per policy.

Provide education regarding the proper ambulation, transferring to wheel chair/bed/vehicle, and the proper restraining process of clients to co-workers/caregivers.

Work closely with the Charge Nurse of the patient regarding patients’ conditions, needs and of any medical changes.

While maintaining clients’ confidentiality, provide thorough documentation of patients’ care as indicated by the health care organization’s policies.

LVN duties and responsibilities while working at an Adult Daycare are but not limited to the following: providing hands-on care for a special population of clients such as clients with mental retardation, schizophrenia, personality disorders, OCD, paranoid disorders, hallucinations, post-traumatic stress disorder, passive aggressive disorders, and anxiety disorders.

Care for clients with chronic illnesses such as but not limited to the following: Rheumatoid arthritis, gout, Parkinson, pressure sores, hypothalamus, HIV, dementia, hepatitis, HTN, hypothyroid, Diabetes I, and II, and asthma. Additional care of clients are as follows: brain injury, seizures, wound care, burns, and patients with ulcers.

Administer prescribed medications, subcutaneous injection, and intramuscular. Insert and provide care to urinary catheter, colostomy care, and wound care.

Continual observation of patients to include charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, side effects, pain relief and ensuring patients are compliant with medication regime.

Ensure client care and treatments are provided by monthly with assessments and vital signs (temperatures, blood pressures, pulse, heart rates, and weight) to include the clients’ overall status.

Teach clients, families, and employees with the proper wound dressing and care, massaging, and health issues as needed.

Coordinating and providing teaching of employees regarding infection control and risk factors, to include the value of continuing education topics needed for employees working at adult daycares.

Consult with, utilize, and make referrals to appropriate community agencies and health care resources to provide continuity of care.

Working with multidisciplinary services such as social workers, case-manager, physicians, transportation employees, clinics, private offices, Adult Protective Services (APS), Mental Health Mental Retardation, Aging and Disability Services, Medicaid services, and other indicated resources needed for the health and safety of the clients. Practice, recognize, and maintain professional boundaries of the nurse-patient relationship. Comply with mandatory reporting requirements of Texas Board of Nurses Nursing Practice Act.
