Assist disabled individuals apply for entitlements such as SSI,SSD, Medicaid, Medicare and Food Stamps while living in transitional housing for 120 days until approval from their Psychiatrist releasing them back into the community, in which at that time they will be referred to a mental health facility to provide outpatient continual care, long term case management and medication delivery.

Provide individual and family counseling, mentoring for young boys in need of male role model, organize and monitor supervised visits per Individual Support Plan formulated by DHR caseworker. Provide transportation to and from Biological / Foster parent home per court order.
