As a nurse I performed total patient care, which included the following: head to toe assessments-listening to lungs, heart, bowels sounds, and performing neurological check on patients (orientation of date, time, year, checking patient’s eyes for reaction and dilation), Check patient’s peripheral circulation and pulses. Also to provide care for many different types of patients afflicted from either diseases and/or surgeries-Chronic Obstructive pulmonary disease (COPD), Congested Heart Failure (CHF), Pneumonia, Cancer, Diverticulitis, Gastro enteritis, Angiograms, Angioplasty, Colostomy, and Hernia repair. My patient’s ages ranged from 1week pediatric patient to 103 geriatric patients. As a nurse it requires you to work with patients that are hearing impaired, blind, senile, bi polar, mentally challenged, confused, and withdrawing from drugs and or alcohol. As a nurse I administered patients medication by different routes-orally, sublingual, subcutaneous, Intramuscular, Intravenously. As a LPN I am qualified to start, flush, care for, hang, and run Piggy back medication through IVs. As a LPN I am qualified to change IV tubing, and to discontinue IVs, I have set up and maintained patient controlled analgesia (PCA) pumps. As a LPN I have assisted physicians while they were performing Thoracentisis, and spinal taps at patient’s bedside. Another responsibility in the nursing field is to check laboratory results and notified doctors of critical lab values pertaining to Complete Blood Count (CBC), B-type Natriuretic peptide (BNP), Basic Metabolic Panel (BMP). I am also qualified to administer Accu checks to determine patients’ blood glucose levels-if above 400 or below 70 the physician would be notified. If the blood glucose level was between 70-400 I would administer the proper amount of Insulin units, if required, according to the Medication Administration Record (MAR), for the patients with low blood sugar levels, (less than 70) would determine if the patient was symptomatic or asymptomatic. If the patient presented with symptoms of lowered conscious levels, profuse sweating (diaphoretic), lethargy, The RN would administer Amp D50 via IV push, if the patient was able to eat or drink the following-drink orange juice with extra sugar, milk, eat- peanut butter and crackers, a sandwich would be given. I performed vital signs either manually or by machine. Would chart on patients on these different areas- rounding, pain, and level of consciousness, bed position, restroom break, respiration any need that the patient requested, every two hours. Ozark Medical Center used the meditech computer charting system, and our facility had the pixis for medication administration.

Another job that I performed at the hospital was Unit Secretary- in this position it requires staff to answer phone calls, admit new patients into the Meditech computer system used by Ozark Medical Center. Also to put together physical charts for new patients being admitted. It also required placing physician orders into the computer, discharging of patients, provide discharge care notes to the patients. Another duty that this position required -putting the correct paperwork for the physicians to use into each individual patients charts (stuffing the charts), also to place Laboratory results under the correct file in each chart. Another requirement was the scheduling of doctor appointments for patients being discharged, also the inpatient and outpatient tests and surgeries by going through central scheduling, would notify Social Services, and Case Management of patients or physician’s needs.

Duties: When I was hired at the Riverways Home Health & Hospice office, one of my first positions with the company was to perform End of Life Care (known as continuous care) for individual patients at the end stages of cancer, and to give support to their families while they were dealing with this very emotional time in their lives. This care was provided so that the patient who wanted to pass away in their own home would have the specialized care they needed. The main focus for my position at this stage of care was to control pain and provide comfort to the patient and their family. I administered pain medication either orally in liquid form that was administered sublingual or by topical gel medication administration on the patients I also administered sublingual atropine medication to decrease fluid secretions that built up in patient’s throat and lungs do to patient inability to cough. This is also known as the death rattle. I would attempt to inform the family members who wanted to be in the room with their loved one that the time was near, so they could say their final goodbyes. I also provided after care to the deceased, I would notify the coroner of time of death would properly dispose of narcotics, would call the funeral home. Would finish up with any and all paperwork needed to be done. Before leaving would make sure that the deceased family members support system was in place as well as it could be.

The Second position that I performed for Riverways was in their Support Services department, where I was assigned to one patient to provide care for. This patient had Gastroschisis at birth, which was not repaired at that time. A pouch was made using skin grafts from the back and buttocks and later on in life his arms and legs, this pouch is known as the omphalocele, and the patient also had a trach. This patient required oxygen therapy at all times. I performed head to toe assessments on the patient daily, I performed tracheotomy care, I suctioned the patient’s trach, I administered oral medication, and I insured that the patient’s oxygen levels were 95 percent and above. I kept the patient safe during the hours of my assigned days. I drew up patient’s liquid oral medical into syringes, labeled them for use during the day. I administered respiratory therapy via the patient’s trach canula of xopenex inhalation medication via a nebulizer machine and I accompanied the patient to and from school; I made sure the patients consumed adequate amounts of nutrition to promote growth and health; I kept in contact with patient’s regular family physician and his pediatrician. And at times I assisted his teacher in educating him and his classmates whenever she requested the help.

Duties: I performed many different duties at the Mountain View Fabricating facility, do to my ability and willingness to do whatever was asked of me by my superiors, but my main position that I held during my employment at Mountain View Fabricating was being in charge of the operating and running of the thirty four individual Rotorary Tuning Station known as the -RTS-machines, these machines programmed coin acceptors to recognize different types of U.S. and International coins. The coin acceptors were programmed to recognize the different amounts that each coin being deposited was worth- for example-nickel, dimes, and quarters for the U.S. vending machines acceptor. These coin acceptors also recognizes the proper amount of money that was needed for each individual purchase and the ability to administered the proper amount of change due to the consumer from that particular machine and purchase. I was responsible for loading and unloading theses 34 machines in a timely manner to achieve quota each hour and, also to properly label the front and top of each individual programmed acceptors, then load them into their proper totes, to fill each individual order that was given for each hour and for each day. I also held the office of president in the committee that dealt with the problem solving on the programming issues that pertained to the U.S. and the International coin acceptors and the Honorary Tuning Station machines. I also had the responsibility of instructing and teaching other new operators how to perform this job.

I also worked in Gloria Dykes department which dealt with the building of different types of paper money acceptors and credit/debit card acceptors. These acceptors recognized one dollars bills, fives dollar bills, tens dollar bills, and twenty dollar bills from each other. The paper money acceptors are used alongside coin acceptors in vending machines. In this department I helped assemble small motors, which were used in the paper money and credit/debit card acceptors, by soldering different types and lengths of wires to the proper area required for each individual motor. I also worked in an assembly line to build paper money acceptors and credit/debit acceptors, which required team work, helping and dealing with different types of personalities in order to achieve our individual assembly lines goal for each hour in producing quantity and most importantly quality parts. I was chosen by my supervisor Mary to be a backup fork lift driver, which required me to take a class and pass a written exam and driving test for licensure and certification. Which I passed these test on the first attempt. I also worked in the shipping department; where I filled orders according to the invoice, I also packaged the different products for shipping. I was voted employee of the month for my perseverance and dedication by my superiors on my job performance on the RTS machine.
