In addition, use form flow to type required data for the DD Form 93 (Record of Emergency Data), DD Form 8285-SGLV (Request for Life Insurance), DD Form 8286-SGLV (Service member’s Group Life Insurance Election and Certificate), DD Form 8286A-SGLV (Family Coverage Election). Verify all data and type any revisions, additions and deletions to their records and files in record of all the military and civilian personnel mobilizing/demobilizing that process, through the Deployment and Reconstitution Tracking Software (DARTS). Scan soldiers’ DD Form 8286-SGLV and DD Form 93 (Record of Emergency Data) onto iPERMS
