Case management that involved senior (60+) independent come to ICSC for socialization and learn to eats healthier. Responsible for making contact with client at least one a month; help senior to translate document and health classes. Responsible for the senior exercise program; conducts a meal site survey. Observes and documents client’s behavior at the meal site and providing valuable feedback to the senior nutrition program manager. Maintaining a safe, secure drug and alcohol free environment; conducts monthly interviews with the seniors of how to improve the qualities services of the meal site program. Participate monthly meeting with the providers and work closely with the Pierce County Health Department on monthly health classes’ presentation. Responsible for helping clients contact the provider Tacoma Housing Authority (THA), Immigration services, Food Stamp and School; help the family located resources in the community; attended Domestic Violence training in Seattle.
