Manage large groups of residents, provide role-modeling, build and maintain relationships, hold residents accountable for their actions, and teach responsibility. Observe, evaluate and record resident’s behavior and provide documentation in reports.

Internship - Providence Newberg Medical Center

Supervisor was Sandy Smith, LCSW. Operated as an acute care manager in which I assisted in guiding patients through the continuum of care. This included but was not limited to screening for risk (medical, psycho-social, and economic), facilitating caregiver support groups, transitional planning, connecting patients to appropriate levels of care and continuity of care, care education, community referrals, and ongoing monitoring for plan effectiveness and continued need of services.
