iTerms of Reference for appointment of Epidemiologist at District HQDISRICT HOSPITAL,COCHIN (The Epidemiologist will report to District Surveillance Officer (DSO) - IDSPIntegrated Disease Surveillance Project (IDSP) was launched by Hon’ble Union Minister of Health & Family Welfare in November 2004. It is a decentralized, State based Surveillance Program in the country. It is intended to detect early warning signals of impending outbreaks and help initiate an effective response in a timely manner. Major components of the project are : (1) Integrating and decentralization of surveillance activities; (2) Strengthening of public health laboratories; (3) Human Resource Development – Training of State Surveillance Officers, District Surveillance Officers, Rapid Response Team, other medical and paramedical staff; and (4) Use of Information Technology for collection, collation, compilation, analysis and dissemination of data

For Project implementation, Surveillance Units have been set up at Central, State and District level. Surveillance Committees at National, State and District levels are monitoring the Project. Currently linkages are being established with all State Head Quarters, District Head Quarters and all Government Medical Colleges on a Satellite Broadband Hybrid Network. The network on completion will enable 800 sites on a broadband network of which 400 sites will have dual connectivity with satellite and broadband. This network enables enhanced Speedy Data Transfer, Video Conferencing, Discussions, Training, Communication and in future e-learning for outbreaks and program monitoring under IDSP. A 24X7 call center with toll free telephone no 1075 accessible from BSNL/MTNL telephone from all states is in operation since February 2008. This receives disease alerts from anywhere in the country and diverges the information to the respective State/District Surveillance Units for verification and initiating appropriate actions wherever required.

Under IDSP data is collected on a weekly (Monday–Sunday) basis. The information is collected on three specified reporting formats, namely “S” (suspected cases), “P” (presumptive cases) and “L” (Laboratory confirmed cases) filled by Health Workers, Clinician and Clinical Laboratory staff. The weekly data gives the time trends. Whenever there is a rising trend of illnesses in any area, it is investigated by the Medical Officers/Rapid Response Teams (RRT) to diagnose and control the outbreak. Data analysis and action are being undertaken by respective districts. Emphasis is being laid on reporting of surveillance data from major hospitals both in public and private sector and also Infectious Disease hospitals. The compilation and disease outbreak alerts has been started recently. On an average 10-15 outbreaks are reported every week to Central Surveillance Unit, IDSP.

Greener areas in the project are Non Communicable Diseases Risk Factor Survey currently being conducted in the states of Andhra Pradesh, Tamil Nadu, Kerala, Maharashtra, Madhya Pradesh, Uttarakhand and Mizoram. The survey is to be repeated every 3 years and all states to be covered in cycles. Urban Surveillance activities have begun in Kolkata Municipal corporation area and due to begin in Mumbai. Also being set up is a network of 7 major Infectious Disease Hospitals located in different regions of the country for rapid sentinel surveillance. IDSP is supporting activities related to Avian Influenza under IDSP with total outlay of Rs. 20.85 crores for three years (2006-09) for Human Component. A networking model has been developed with 10 laboratories and additionally ICMR with its four branch laboratories.

Objective of the assignment

The main objectives of the assignment are to:

a. Provide technical support to State Surveillance Officers to implement IDSP and ensure operational continuity for the program.

b. Provide oversight for data collation, reporting, analysis and local action including outbreak investigation. Administer collection, compilation and analysis of passive surveillance data not only from peripheral health institutions but also from hospitals and colleges in the district.

c. Organize Rapid Response Teams for undertaking prompt outbreak investigations. Feedback to lower level institutions on reporting, trends and outbreaks. Local media monitoring.

d. Support capacity building by organizing training for state IDSP staff, monitoring training impact on the field and assessing emerging training needs

e. Liaison with other departments that have relevance to disease surveillance (Animal Husbandry, Women and Child Welfare, Panchayat Raj etc.) and Organize activities to strengthen partnerships with Medical Colleges, NGO sector & Private sector in disease surveillance and control of diseases. Monthly reviews and compilation of periodic reports and plans under the project.

f. Support the organization of annual state dissemination meetings on disease surveillance and preparation of “state annual disease surveillance report” by January every year.

g. Provide technical support for National Vector Borne Diseases program (NVBDCP) through supporting fever surveillance and oversight for the line listing of vector borne diseases

Job Responsibilities:

• Organize and monitor timely collection of data from all reporting districts. Administer collection, compilation and analysis of passive surveillance data not only from peripheral health institutions but also from hospitals and colleges in the state.

• Analyse surveillance data and prepare weekly surveillance graphs and charts

• Identify outbreaks of diseases targeted in IDSP for all reporting districts using triggers agreed with Central Surveillance Unit.

• Collect monthly summaries of the disease situation from the district surveillance units.

• Prepare and send monthly summaries of the disease situation to the Central Surveillance Unit and Regional Project Coordinators .

• Initiate outbreak investigations promptly following the standard operating guidelines of IDSP

• Coordinate movement of Rapid Response Team & participate in all outbreak investigations

• Ensure timely submission of FIR (First Information Report) and detailed Outbreak Report to SSU/CSU and Regional Project Coordinators.

• Ensure timely submission of annual project report and annual surveillance report of DSUs, prepare and timely submit annual project report and annual surveillance report for the SSU to CSU and Regional Project Coordinators.

• Support effective operational integration of disease control efforts based on the surveillance data

• Coordinate involvement of Medical Colleges, Private Sector, Community and Media in surveillance activities

• Provide regular feedback to all reporting districts on disease trends and outbreaks

• Organize, coordinate & monitor training of state/district staff under IDSP.

• Identify emerging training needs at the state/district and revise training calendars accordingly.

• Organize dissemination of training manuals and materials for training courses.

• Assist in organizing independent evaluation studies under IDSP and its outcome

• Supervise Data Manager and Consultant Finance/Procurement appointed under IDSP to ensure timely submission of quality information required in relation to data and finance/ procurement matters

• Monitor proper use of EDUSAT facility/IT networking towards data transfer, training, e-conferencing.

• Coordinate regular meetings of State/District Surveillance Committee and assist in inter-sectoral coordination for effective IDSP implementation.

• Organize regular meetings of IDSP stakeholders.

• Make supervisory visits to DSUs to monitor implementation of project activity

• Support state surveillance officers in carrying out other works related to effective implementation of IDSP

DUTIES AND RESPONSIBILITIES

Training: Conduct outreach to hospitals, providers and other partners regarding syndromic surveillance systems; develop and deliver training for hospitals, providers and other partners regarding syndromic surveillance systems; identify resources and administrative steps needed for project completion; and develop project plan in consultation with PSET Epidemiologist 2.

Advise local health department personnel on control of communicable diseases, including design of investigations and analysis of data, including ACDP day-call epidemiologist. Urgent Epidemiology Response Team (UERT) member that responds to communicable disease outbreaks, including travel to outbreak sites to lend expertise. Disease investigation and control assistance to county disease investigations. Orpheus Tech Team.

Provide epidemiologic consultation to and serve as liaison for local health department staff, healthcare providers and other state and local agencies regarding public health emergency preparedness planning; serve as a consultant for selected communicable diseases, including surveillance and investigation, development of fact sheets, investigative guidelines, and CD Summary articles.

Provide epidemiologic consultation to and serve as liaison for local health department staff, healthcare providers and other state and local agencies regarding the statewide syndromic surveillance system; maintain data-use agreements and respond to ad hoc requests for surveillance data. Plan outbreak investigations and other epidemiologic studies, including identify study populations, determining appropriate study design and data-gathering instruments, data collection and analysis, preparation of summary reports; review, analyze and interpret surveillance data for conditions of public health significance; assist in developing Quality Assurance plan of local health department (LHD) and state electronic surveillance systems.
